Fertility concerns can have many causes affecting the woman, the man or both partners. A structured medical assessment can help clarify the individual situation and identify appropriate next steps. Univ.-Doz. Dr. med. Nicolas Zech, PhD offers personal guidance drawing on experience in reproductive medicine, hormonal assessment and evidence-informed care.

When should you seek a fertility assessment?
If pregnancy does not occur despite regular unprotected intercourse, a medical assessment may be helpful. Timing depends on age, menstrual cycle, medical history and known conditions. Earlier advice may be appropriate for irregular or absent cycles, endometriosis, PCOS, previous surgery, recurrent pregnancy loss or a known male fertility factor.
Fertility tests for both partners
Fertility involves both partners. Depending on the situation, an assessment may include medical history, hormone and cycle evaluation, ultrasound findings, examination of possible causes such as endometriosis or PCOS, and male fertility assessment including semen analysis. Existing laboratory results and records from previous treatment should be reviewed before unnecessary tests are repeated.
Treatment options for infertility
Treatment depends on the identified cause, age, duration of infertility and personal circumstances. Not every couple needs assisted reproduction. Options may include treating an underlying condition, cycle monitoring, medication, intrauterine insemination or referral to and cooperation with a specialised fertility centre.
Intrauterine insemination
Prepared sperm cells are introduced directly into the uterus using a thin catheter. Whether insemination is suitable depends on the individual diagnosis.
IVF – in vitro fertilisation
During IVF, eggs are collected following ovarian stimulation and combined with sperm outside the body. If fertilisation and development occur, an embryo may be transferred to the uterus.
ICSI – intracytoplasmic sperm injection
ICSI is a form of assisted reproduction in which one sperm cell is injected directly into an egg in the laboratory. It may be considered for certain male fertility factors or other specific indications.
